Case Description
74-year old presents with:
[A] #31 crowned, with suspected abcess. Family dentist took x-rays and recommeded root canal and refered patient to endodontist. First endodontist recommended immediate root canal, and treatment of suspected abcess No action taken by patient Second endodontist said no action re. a root canal and suspected abcess were necessary.
[B] #18, crowned as one of two anchor teeth in a three-tooth, 50-year old bridge. Family dentist diagnosed decay under the bridge. First endodontist did not look at it. Second endodontist recommended three implants versus decay remediation with necessary periodontal treatment, and bridge replacement

If the scenario is conveyed/understood accurately :A) If #31 is asymptomatic, take another Xray of #31 after approx. 10 days-2 weeks and let it decide whether or not to proceed w/ root canalB) The 2nd endodontist''''s opinion seems reasonable but might be able to salvage front tooth of the old bridge (#20) w/o treatment and just plan 2 implants. The replacement bridge option involving periodontal treatment might be less predictable but doable.Good luck

No dentist can give you a recommendation without seeing xrays and examining the patient''''''''s mouth. A 50 year old bridge would be susceptible to recurrent decay and if the decay is significantly below the bone level, a crown lengthening on a second molar probably would be futule and implants would probably be the ideal option. A removable partial denture may also be feasible. A periodontist would be the one to give the best decision about a crown lengthening since it involves the supporting structures (bone and gingiva) and the root of the teeth.Again, an opinion about #31 would depend upon records. You should expect each dentist to explain why they recommended what they did and the pros and cons of the decisions. If I knew the exact dentists involved and saw the clinical findings, then I would be able to rate their relative clinical accumen.

hire patient #A: If there is an abscess evident on the x-ray it should be treated even if there is no clinical symptoms. Although, I would have to look at the x-ray and evaluate patient for the ultimate recommendationre patient #B: Dental implants are certainly the beneficial over bridges in many ways. Current tooth replacement therapy involves implants in majority of cases. Bridges, although an option, have many disadvantages and rarely meet patients'''' desires and needs.
