Case Description
I have full dentures both upper and lower. The lower denture is secured by four implants in the front of my mouth. Is it possible to have false teeth without the denture covering the top of my mouth and to improve the lower denture which I feel takes up a lot of room although it fits well and works well.
I am assuming because I have had dentures for a number of years that I would require bone grafts both on my upper and lower mouth to replace deterioration.

That really depends upon the type of implant framework that you have on the upper and their placement. If they are robust enough to put a meal framework on, then you can have an implant overdenture which is open on the roof of your mouth. The lower denture may or may not be able to be helped. Again implants would be the route to go on the lower if you already do not have them where they clip on to the lower jaw too.

Bone grafts without added implants will not help you as the new bone will atrophy within a few years if you do not place implants; the implants are what will preserve the bone by stimulating it similarly to the way real teeth stimulate bone if the periodontal tissues are kept clean.Added implants to secure a permanently fixed bridge is a much more secure and lasting treatment than you have on the mandible. This treatment can also usually be done on the upper jaw. Some bone grafts around the new implants might be necessary and they can be done in the periodontal office using local or general anesthesia.

Many of our patients do not need a bone graft but it would depend on the amount of remaining bone you have in your upper arch.
